New Patient Assessment Process
Quantum Health Practice welcomes new patients daily. The first step in pain management, addiction treatment, and recovery starts with you, and we are happy to guide you on your mission to become your best self.
Please read the guidelines below on our New Patient Intake & Assessment Process
New Pain Management Patients
New patients coming from any other facility will need to bring the following items with them on their first visit:
- Past Medical Records
- Copies of Films (MRI, PET, etc.)
- Pharmacy Profile (last 6 months)
- $300, Cash or Credit, for initial visit (subsequent visits are $200)
New Addiction Treatment Patients
Before any treatment for addiction can be started, new patients can expect:
- A full review of their past medical history
- Tests to determine if/what patient is currently addicted to
- Treatment recommendation will then follow after successful initial assessment
Quantum Health Practice will then administer a comprehensive assessment for new pain management and/or addiction treatment patients which will include the following:
- Q&A session with physician to target where the pain is located, determine the pain intensity level, and understand the obstacles faced with pain during everyday activity
